We are pleased to announce a new feature which allows administrators to submit care navigation requests on behalf of patients. This enhancement ensures that all patients, including those who may not be digitally proficient, have equal access to healthcare services. The intuitive submission process is designed to streamline the way care requests are handled, benefitting both healthcare professionals and patients alike.
Working across multiple teams often presents a unique challenge of having to spend time just to understand what has already happened, rather then focusing on delivering care. With our new care history timeline you can see all the relevant information in one simple interface. Spend less time chasing people up and spend more time caring for patients.
As part of Hero’s care history timeline all the changes to a request can now be accessed in one place. In the care history timeline you can see:
When a request was made.
When the status of the request was change and by who.
Who has been and is assigned to the notification.
If the request has been saved to the patient EHR record.
And if the request has been responded to.
Primary care teams are under enormous pressure from patients and NHSE to perform more efficiently and provide care to more and more people without compromising the quality of care. This means there is little time to review their previous work or analyse how the process could be streamlined.
Our care navigation dashboard means teams have these insights at their fingertips without configuration or setup. The insights from the dashboard can help practice teams find out how to optimise the process and manage their resources more effectively.
