We have updated the Hero platform to allow you to organise and manage your intake forms and questionnaires. This separation between form types aims to more clearly differentiate the use of each form type and improve your practice’s workflows.
Intake forms have been built off the same architecture as questionnaires, which allows you to access all the functionality previously available through questionnaires. This includes customisable questions, medical conditions & observations, and conditional logic.
Intake forms have been developed as a dedicated solution to capture structured patient data as part of the appointment booking flow, compared with questionnaires which can still be used when messaging patients. With this change intake forms can be created and edited through the settings page under appointments and associated with specific appointment types. Responses to intake forms will be accessible in the inbox via the appointment & intake form filter as seen below.
